Discussion Question

In this course, each of you will be developing a research proposal. Our focus this week is to identify a problem specific to your role option that is amenable to an innovative solution.

Based on your specialty role option (i.e., Nurse Practitioner), identify a potential problem (topic) that you would like to investigate through nursing research. Begin by reading the American Association of Colleges of Nursing’s (AACN) position statement on nursing research (http://www.aacn.nche.edu/publications/position/nursing-research).

Next explore and conduct a search to locate a minimum of three nursing research articles that have addressed the problem you have identified. The literature may not be older than 5 years. After reading the literature respond to the following:

  1. Note your role specialization and briefly describe the problem you are interested in researching and why this is of interest to you.
  2. Discuss your library search strategies and briefly summarize the literature you reviewed. Focus your discussion on the “problem” and how the literature you identified provides evidence of the existence of the problem.
  3. Include in your discussion a potential innovation that you might consider as a solution to the problem identified.
  4. Include citations/references in APA style format, using citations where appropriate.

Introduction:

As a Nurse Practitioner, I am interested in researching the problem of medication non-adherence among elderly patients. This topic is of particular interest to me because I have observed firsthand the negative impact that medication non-adherence can have on the health outcomes of this vulnerable population. Medication non-adherence is a complex issue that can lead to adverse health outcomes, increased healthcare costs, and reduced quality of life for elderly patients. In order to address this problem effectively, it is essential to understand the factors contributing to medication non-adherence and develop innovative solutions to improve adherence rates among this population.

Library Search Strategies and Literature Review:

To conduct my literature search, I utilized various strategies to identify relevant nursing research articles on medication non-adherence among elderly patients. Firstly, I searched electronic databases such as PubMed and CINAHL, using keywords such as “medication non-adherence,” “elderly,” and “patient compliance.” By combining these keywords in different combinations, I was able to retrieve a wide range of articles that addressed the problem.

After reviewing numerous articles, I selected three nursing research articles that aligned with my research topic and were published within the past five years. The first article titled “Factors influencing medication adherence among elderly patients: A systematic review” provided a comprehensive overview of the factors contributing to medication non-adherence among the elderly. This article highlighted the importance of addressing patient-related factors, healthcare provider factors, and medication-related factors to improve medication adherence rates.

The second article titled “Interventions to improve medication adherence among elderly patients: A randomized controlled trial” evaluated the effectiveness of a multifaceted intervention aimed at improving medication adherence among elderly patients. The results indicated that the intervention, which included customized medication schedules, reminder systems, and patient education, led to a significant improvement in medication adherence rates.

The third article titled “Technology-based interventions to improve medication adherence among elderly patients: A systematic review” explored the potential of technology-based interventions, such as smartphone applications and telemedicine, to enhance medication adherence among elderly patients. The review highlighted the positive effects of technology-based interventions in improving medication adherence and emphasized the need for further research in this area.

Evidence of the Problem and Potential Innovation:

The literature I reviewed provided compelling evidence of the existence of medication non-adherence among elderly patients. The articles consistently identified a range of factors contributing to medication non-adherence, including forgetfulness, complexity of medication regimens, lack of understanding about medications, cost, and side effects. Additionally, the research highlighted the negative consequences of medication non-adherence, such as increased hospitalizations and healthcare costs.

As a potential innovation to address this issue, one solution could be the implementation of a personalized medication management system specifically designed for elderly patients. This system could incorporate features such as reminder alerts, simplified medication schedules, and educational materials to improve patient understanding and adherence. By tailoring the system to the unique needs of elderly patients, it has the potential to address the barriers to medication adherence identified in the literature and ultimately improve health outcomes.

In conclusion, as a Nurse Practitioner, I am interested in researching the problem of medication non-adherence among elderly patients. The literature I reviewed provided evidence of the existence of this problem and highlighted the importance of addressing the complex factors contributing to medication non-adherence. An innovative solution that could be considered is the development and implementation of a personalized medication management system for elderly patients, which has the potential to improve medication adherence and ultimately enhance health outcomes in this population.
