Plan and develop a plan targeting each level of prevention, for example, for Primary Prevention a plan to promote a safe sex program in communities with a high percentage of teenage pregnancies. Describe the plan for implementation of one of these initiatives and identify barriers to implementation and how to overcome the barriers. 

Lastly provide a detailed evaluation of your program.
