Instructions

Imagine you are working as a safety and quality partner for a health care organisation. You have been

asked to prepare a report and presentation to the executive team to demonstrate the importance of a
safety and quality agenda, emphasising the value of organisational culture. The approach has been set
and it has been decided to show the value of a safety and quality agenda by examining a major failing in
another health care service.

As such select one of the following public inquiries:

 Bristol Royal Infirmary

 Bundaberg Inquiry

 King Edward Memorial Hospital

 Royal Liverpool Children’s NHS Trust Inquiry

 Shipman Inquiry

 The Allitt Inquiry

 An inquiry of your choice that relates to a health service (check with Sharon if not sure)
And summarise the inquiry in a 2,000 word report (+/- 10%), covering the following areas:

 What happened?

 How it happened?

 Who was involved?

 Why did it go undetected for a period of time?

 What recommendations were made?

 What group(s) of professionals were involved?

 Who discovered the problem?
This is not an exhaustive list of questions; the report can be tackled any way you feel will give the best
overall summary and convey the quality and safety implications for health care services.

Once the report is prepared, create a supporting slide deck of no more than 12 slides (including a

title/introductory slide and a wrap up slide) to summarise the report, focusing on the key findings and
recommendations. This slide deck will be presented to the executive to support the report, so it should be
realistic, concise, clear and well formatted.

Support the slides with notes to include the detail that will be discussed and use the slide itself for the

simple key messages. Use supporting images and graphics where appropriate. See the section on
structure below for more information on how to submit the report and presentation as single document.

Please ensure that appropriate APA 7th edition referencing, as per university standards, is included for
both the report and presentation as needed.

Structure

A single document must be submitted to Turnitin, therefore the following outline structure is suggested:

1. Cover Page

2. Contents
3. Part A: Report
4. Part B: Presentation

5. References
As such the slides will need to be added into the main document ensuring the transcript/slide notes are
also copied/visible. The simplest approach will be to copy a slide from PowerPoint and paste it into Word
and then copy the slide notes and manually paste them under the copied slide.

Grades will be available via Blackboard within three weeks of the submission date.

This assignment is worth 35% and is due by 9.00 am AWST on Friday the 18th October 2024.
